What HIPAA Breach Notification Rule (45 CFR Parts 164.400-414) - Unsecured Protected Health Information Breach Response requires
This rule requires HIPAA-covered entities and their business associates to provide notification to affected individuals, the Secretary of Health and Human Services (HHS), and, in some cases, the media following a breach of unsecured Protected Health Information (PHI). As per 45 CFR § 164.404, individual notifications must be provided without unreasonable delay and in no case later than 60 calendar days after discovery of the breach.
