EU AI Act Regulation 2024/1689 Article 73 requires providers of high-risk AI systems placed on the Union market to report serious incidents to the market surveillance authorities of the Member States where the incident occurred - a serious incident being any incident or malfunction of a high-risk AI system that directly or indirectly leads to the death of a person, serious damage to a person's health, serious damage to property or the environment, a serious and irreversible disruption of a critical service or infrastructure, or a violation of obligations under Union law protecting fundamental rights - with the incident report submitted without undue delay and in any event no later than 15 days after the provider becomes aware of the incident.
